Provider Perspectives on Remote Inpatient Teleneurology Consult Service

To characterize benefits and challenges of inpatient virtual teleneurology consultations by ordering providers and consulting neurologists.
Telestroke has provided expert-level virtual care since the 1990s; however, inpatient teleneurology has not been widely integrated. Wake Forest Baptist Medical Center (WFBMC) implemented inpatient teleneurology consultations to provide expert-level neurology consultation virtually at three satellite campuses lacking in-person neurologists. We characterize feedback of ordering providers and consulting neurologists during the pilot phase of implementation.
Ordering providers and consulting WFBMC neurologists involved in inpatient teleneurology consults were surveyed via email. Retrospective chart review summarized consult characteristics.

Ordering providers were sent a survey about their experience, and neurologists completed a survey after each consult. Ordering provider response rate was 46% (11/24), and neurologist response rate was 74% (17/23 completed consults). Ordering providers were system leaders (18%), physicians (64%), and APPs (27%, one participant selected two roles). Of these, 91% agreed that teleneurology consults added value to patient care; 100% agreed that availability of consults was essential to keeping patients in their community and preventing unnecessary transfers. Providers were asked about course of action had teleconsult been unavailable; 36% of ordering providers believed transfer would have occurred compared to only 6% of consulting neurologists while 18% of ordering providers and 40% of consulting neurologists believed phone advice would have been sufficient.

The most common suggested improvement was ease of consistent communication with on-call teleneurology providers. Neurologists reported connectivity challenges and confusion deciphering reason for consult in the electronic system.

Ordering providers and consulting neurologists reported value of inpatient teleneurology consultation. While neurologists typically felt that a phone call would have been sufficient, hospitalists saw additional benefit of teleneurology in avoiding transfers, preventing harm, and improving diagnostic capability and length of stay. Additional feedback will be gathered to further expand telehealth services.
