Preventing Loss of Independence through Exercise (PLIe): An Integrative Exercise Program for Individuals with Dementia

Traditional exercise programs have been found to improve physical function in individuals with dementia, but less is known about the effects of exercises that integrate functional movements with mindful body awareness, which may also impact cognitive function.
The PLIe program incorporates elements of physical/occupational therapy, yoga, Tai Chi, Feldenkrais and Rosen Method and focuses on movements required to maintain function (e.g., standing from sitting) in a social group environment. An 18-week (45 minutes, 2-3 days/week) pilot-test was performed in 11 individuals (6 PLIe, 5 usual care, with group assignment based on sex and days of attendance) at a day care program in San Francisco, CA. Pre- and post-intervention data were collected by blinded assessors. Measures included the Alzheimer's Disease Assessment Scale-cognitive subscale (ADAS-cog), 3-meter gait speed, and Caregiver Burden Inventory (CBI). Measures of effect size were calculated. Adverse events were monitored through biweekly caregiver check-ins.
PLIe and usual care groups were similar at baseline. ADAS-cog scores improved 4.6 points (27.0 to 22.4) with PLIe and worsened 2.4 points (23.7 to 26.1) with usual care (effect size, 0.76). Gait speed improved 0.3 seconds (5.7 to 5.4) with PLIe and worsened 0.8 seconds (5.4 to 6.2) with usual care (effect size, 0.53). CBI scores decreased 5.5 points (29.8 to 24.3) with PLIe and increased 1.8 points (32.5 to 34.3) with usual care (effect size, 0.51). There were no serious study-related adverse events.
This pilot study provides preliminary evidence that the PLIe program may improve cognitive function, physical performance and caregiver burden when compared with usual care, with an effect size comparable to or exceeding currently available medications. A larger randomized, controlled trial is warranted.
