Defining the Neuroimmunology/Multiple Sclerosis Subspecialty: Surveys of Fellowship Training in the United States

To identify the scope of current neuroimmunology training programs and self-reported readiness of recent fellowship graduates for clinical practice.

Neuroimmunology is a rapidly evolving subspecialty encompassing multiple sclerosis (MS), neuromyelitis optica spectrum disorders, autoimmune encephalitis and neuroimmune complications of systemic diseases, among others. New neuroimmunologic diseases are constantly being recognized, testing has become more nuanced, and treatment options are burgeoning. Amidst this increasing complexity, curricula and scope of training vary substantially across US fellowship programs.
We identified fellowship training programs using a comprehensive web search coupled with direct faculty contacts at MS centers. Additionally, we identified current trainees and recent graduates (2014-2019) through fellowship program directors and by reviewing fellowship awardees through the National MS Society. Separate electronic surveys were emailed to fellowship directors and to trainees/recent graduates, which addressed training experience and workforce readiness as well as open-ended questions regarding views on subspecialty standardization.
We identified 69 total fellowship programs and received data from 64 (93%). We contacted 163 trainees/recent graduates and obtained responses from 80 (49%). Fellowship directors and trainees/recent graduates independently reported the expectation that training should extend beyond MS to include other neuroimmunologic conditions. However, exposure to non-MS conditions varied across training programs. Some recent trainees reported inadequate exposure to conditions like autoimmune epilepsy (49%), vasculitis (27%) and paraneoplastic disorders (16%). Participants valued cross-disciplinary electives. Some felt that increased cross-disciplinary exposure would enhance training and identified rheumatology (45%), basic immunology (44%), urology (38%) and neuro-rehabilitation (38%) as areas of particular interest. Most recent trainees (59%) favored standardizing a neuroimmunology/MS fellowship curriculum.
The scope of neuroimmunology has been expanding, and specialists are expected to be knowledgeable about a spectrum of rare diseases beyond MS. Standardizing a fellowship curriculum will help define the scope of the specialty and ensure standards for clinical expertise.
