Associations of Serum Neurofilament Light Chain with Clinico-Radiological Characteristics in the MSPATHS Network: A Cross-Sectional Evaluation

To examine associations of serum neurofilament light chain (sNfL) with demographics and clinico-radiological characteristics in a large, multi-center cohort of people with multiple sclerosis (MS).
Clinical validation of sNfL, measured using high-throughput assays on widely accessible platforms, in large and heterogeneous MS populations, is a critical step in order for sNfL to be utilized in clinical practice.
In this cross-sectional evaluation, sNfL was measured in baseline serum samples from 6,968 MS and 201 healthy control (HC) participants in the Multiple Sclerosis Partners Advancing Technology and Health Solutions (MS PATHS) cohort, using a novel high-throughput immunoassay (Siemens Healthineers). Age-specific cut-offs of sNfL levels were derived using the HC data, and the 97.5th percentile was defined as the threshold for elevated sNfL. A subset of MS participants (n=3,878) had available standardized MRI acquisitions with volumetric measures. Analyses were performed using multi-variable generalized linear models.
Elevated sNfL was found in 1,202 of 6,698 MS participants (17.2%). Factors associated with elevated sNfL included progressive MS (OR:1.63, 95%CI:1.38-1.92), non-White race (OR:1.43, 95%CI 1.17-1.74), diabetes mellitus (OR 1.89, 95%CI:1.42-2.49), and smoking (current vs never smoker, OR 1.49, 95%CI:1.20-1.85). Age and symptom duration exhibited complex associations with sNfL status, but overall the highest frequency of elevated sNfL was observed in younger patients with shorter disease duration. Higher body mass index was associated with lower odds of elevated sNfL (OR 0.83 per 5 kg/m2 increase, 95%CI: 0.78-0.88). MS participants with elevated sNfL exhibited worse neurological function (walking speed, manual dexterity, and processing speed), lower brain parenchymal fraction, lower thalamic volume, and higher T2 lesion volume (p<0.001 for all).
Elevated sNfL is associated with clinical disability and brain atrophy in MS, and with comorbidities including diabetes and smoking. Future longitudinal analyses of this cohort will be important to further validate sNfL as a biomarker of the MS disease course.
