Title Long-Term Effects of Lower-Sodium Oxybate on Functioning and Work Productivity in Participants With Idiopathic Hypersomnia

Objective To report the long-term effects of lower-sodium oxybate (LXB; Xywav^®) on functioning and work productivity in a placebo-controlled, double-blind, randomized withdrawal study in adults with idiopathic hypersomnia (IH).

Background IH is a debilitating neurologic sleep disorder characterized by excessive daytime sleepiness; symptoms may also include prolonged nighttime sleep and severe sleep inertia. LXB is a new oxybate treatment with 92% less sodium than sodium oxybate (Xyrem^®). LXB was approved by the United States Food and Drug Administration in August 2021 for the treatment of IH in adults.

Design/Methods Eligible participants (18–75 years of age) began LXB treatment with an open-label titration and optimization period (10–14 weeks), followed by a 2-week, open-label, stable-dose period (SDP). Participants were randomized to placebo or continued LXB treatment during a 2-week, double-blind, randomized withdrawal period (DBRWP). A 24-week open-label extension period (OLE) followed. Functional Outcomes of Sleep Questionnaire, short version (FOSQ-10) and Work Productivity and Activity Impairment: Specific Health Problem (WPAI:SHP) questionnaire were evaluated at baseline, end of SDP, OLE week (W)6, W14, and W24/early termination (ET). Results during open-label LXB treatment (W24/ET) are reported here.

Results The study enrolled 154 participants. Mean±SD FOSQ-10 scores improved from baseline (11.8±3.4) to end of SDP (16.7±3.0) and remained stable through OLE W24/ET (17.3±2.7). Mean±SD baseline WPAI:SHP measures were 7.4±15.8 (absenteeism), 55.3±23.0 (presenteeism), 57.2±23.4 (absenteeism+presenteeism), and 65.2±22.4 (activity impairment). WPAI:SHP measures improved during titration/optimization, and remained stable through OLE (absenteeism [mean±SD]: end SDP [1.8±5.7], W24/ET [5.0±12.0]; presenteeism: end SDP [22.1±20.0], W24/ET [20.3±20.4]; absenteeism+presenteeism: end SDP [23.1±21.2], W24/ET [23.5±22.8]; activity impairment: end SDP [26.8±22.5], W24/ET [26.2±23.9]). Treatment-emergent adverse events (≥10%) included nausea, headache, dizziness, anxiety, and vomiting.

Conclusions Sustained efficacy on functioning (FOSQ-10) and work productivity (WPAI:SHP) in adults with IH was maintained throughout the open-label extension period. The overall safety profile was consistent with that of LXB in narcolepsy.
