Treatment of Advanced Parkinson's Disease with Intrajejunal Infusion of Levodopa/Carbidopa Gel - 7 Year Experience

BACKGROUND: In advanced PD the quality of patient's life is decreased due disease and side effects of treatment. Continuous dopaminergic stimulation with IILG may improve patients' quality of life.
DESIGN/METHODS: We report 45 patients with advanced PD who were treated with IILG during the course of their disease. Their satisfaction with IILG was followed and side effects of IILG treatment were closely observed. On a VAS patients were asked to evaluate their subjective change of disease-related quality of life before IILG and at least 3 months after its introduction.
RESULTS: 31 PB patients (15 female, age 68, 8 years +/- 7,3) were followed for 1 - 7 years (mean 3, 5 years +/- 1,9). Their subjective disease-related quality of life improved for 77 +/- 15 percents. 26 (83,4%) of them had side effects related to IILG treatment: pump dysfunction in 20 (64,5%), tubes dislocation in 17 (54,8 %) and local skin infection in 12 (38,7 %) patients. Additional 14 PD patients were transiently treated with IILG of whom eight died (four of sudden death, two due to sepsis unrelated to IILG, one due to brain hemorrhage, one patient choked himself with food, two became psychotic, one developed severe peritonitis early after IILG introduction, one had severe dystonia, one was unsatisfied with the IILG treatment and in one patient's relatives were not willing to collaborate.
CONCLUSIONS: In conclusion, patients demonstrated high degree of satisfaction with the IILG treatment and their quality of life improved considerably. However, many experienced side effects of IILG treatment, mostly related to the pump and tubes. Therefore continuous technical assistance should be available to the patients treated with IILG.
