Chinese or South Asian Ethnicity Is Not Independently Associated with Delayed Treatment in Acute Stroke: Observations from the Vancouver General Hospital Stroke Program

BACKGROUND: Vancouver has a unique population of minority ethnic groups with the largest groups comprised of Chinese and South Asians. With a central referral hospital, it is unclear if ethnicity is a determinant of prolonged DTNT.
DESIGN/METHODS: A retrospective chart review was performed of all patients (n=250) treated with tPA consecutively at Vancouver General Hospital (VGH) from January 2009 to March 2011. Demographic features along with times of symptom onset, arrival, CT scan and treatment with tPA were recorded for Chinese, South Asians and all other ethnic groups ("Others"). Logistic regression analysis was used to determine independent variables associated with a DTNT ?60 minutes.
RESULTS: The final study population included 203 patients with 20% Chinese (n=40) and 14% South Asian (n=29) patients. The mean DTNT for Chinese and South Asians was not significantly different from Others (79 and 59 vs. 70 minutes, p=0.33 and p=0.16 respectively). Mean onset to treatment time was not significantly different between Chinese, South Asians and Others (p=0.09). Patients that were transferred to VGH had a significantly shorter DTNT than those who presented to VGH (52 vs. 81 minutes, p <0.001). In multivariate logistic regression analysis, hospital transfer (p <0.001) was the only factor associated with DTNT< 60 minutes. Ethnicity was not an independent factor associated with a prolonged DTNT (p=0.3).
CONCLUSIONS: Preliminary data did not identify ethnicity as an independent variable in a delay to treatment time in acute ischemic stroke. However, hospital transfer is an independent variable associated with a reduced DTNT and this requires further study as it may alter future treatment protocols.
