[18F] Flutemetamol Amyloid-beta Imaging across the Spectrum of Alzheimer's Disease

BACKGROUND: The PET amyloid imaging with [18F]-labeled PIB derivate [18F]-Flutemetamol have recently developed to be a new novel technology of underlying AD pathology.
DESIGN/METHODS: Thirteen patients with AD, 31 with mild cognitive impairment (MCI) and 19 subjects with healthy control (HC) were studied. All 63 patients underwent 30-min static [18F]-FMM PET, acquired 85 min after injection (193.5 卤 6.6 MBq), and 60-min dynamic [11C]-PIB PET. [18F]-FMM scans were assessed visually by 3 individuals as abnormal or normal cortical uptake, and the standardized uptake value ratios (SUVR) were defined quantitatively in regions of interest defined on co-registered MRI (cerebellar gray as a reference region). The PIB distribution volume ratios (DVR) were determined in same cortical regions.
RESULTS: The visual assessments of [18F]-FMM scans had an increased A? deposition in all of 13 scans with AD patients and 1 of 19 scans with HC subjects, corresponding to a sensitivity of 96.2% and a specificity of 93.3%. These findings were completely same to [11C]-PIB scans. The [18F] SUVR value in whole cortical regions in AD was significantly greater than in 18F]-FMM negative HC (1.81 卤 0.22 vs 1.24 卤 0.09, P<0.01). MCI patients had a bimodal distribution of SUVRs. The SUVRs in 6 patients with [18F]-FMM positive MCI and 25 patients with negative MCI were 1.87 卤 0.27 and 1.19 卤 0.08, respectively. The [18F]-FMM SUVR values in all individual subjects were significantly correlated with [11C] PIB DVR values (r=0.94, n=63, p<0.001).
CONCLUSIONS: [18F] Flutemetamol PET provides images similar to [11C]-PIB PET, and is a reliable imaging biomarker of A? deposition along the continuum from normal cognitive status to AD dementia.
