Post-Stroke Use of Pioglitazone Improves Atherothrombotic Stroke Outcome in Non-Diabetic Abdominally Obese Patients

BACKGROUND: Recent multiple data from animal experiments strongly indicate that thiazolidinediones confer neuroprotection and neurological improvement following cerebral ischemia due to pleiotropic effects. But so far, there are limited clinical data on thiazolidinediones use in patients with ischemic strokes. On the other hand abdominal obesity is associated with insulin resistance and more severe ischemic strokes.
DESIGN/METHODS: This prospective study evaluated 71 non-diabetic abdominally obese patients (class II obesity) aged 67.7卤5.9 years (44% women) with acute ischemic atherothrombotic stroke. Patients were matched for National Institutes of Health Stroke Scale score at admission, cerebral infarcts volume and pre-stroke disability. Patients were randomized into a control group (n=37) and a pioglitazone 30 mg/d group (n=34). Treatment with pioglitazone was initiated within 72 hours after the onset of stroke and continued up to 90 days. Favorable outcome was defined as modified Rankin scale (mRS) score ?2 and Barthel index (BI) ?85 at discharge, at 28 and 90 days. Total mortality rate was assessed once at 90 day.
RESULTS: There was no significant difference in total death rate at 90 day between pioglitazone and control groups (8.8% versus 13.5%). There was no any difference in functional outcomes at discharge. According to mRS, late favorable outcomes were more frequent in the pioglitazone group (OR=3.07; 95% CI 1.09 to 8.21; p=0.056 at 28 day; OR=3.68; 95% CI 1.18 to 10.60; p=0.04 at 90 day). Similar findings were observed with BI (OR=4.43; 95% CI 1.42 to 12.61; p=0.02 at 28 days; OR=4.04; 95% CI 1.22 to 12.11; p=0.03 at 90 days).
CONCLUSIONS: Poststroke use of pioglitazone is associated with improved late outcome in abdominally obese patients with acute atherothrombotic stroke.
