Brain Structural and Functional Abnormalities in Fahr's Disease: Report of Two Cases

BACKGROUND: Bilateral strio-pallido-dentate calcinosis is commonly referred to as 'Fahr's disease'. Clinically, parkinsonism, cognitive impairment, ataxia, sensory changes and pain are the common presentation.
DESIGN/METHODS: Using a 3.0 Tesla scanner, 3D T1-weighted and RS fMRI scans were acquired from 2 patients with Fahr's disease and MWA, 15 patients with MWA and 15 healthy controls. VBM analysis was performed using SPM8, masking the analysis for basal ganglia calcinosis. RS fMRI analysis was performed using the GIFT software and SPM8.
RESULTS: Compared to controls and migraine patients, Fahr's patients had atrophy of the left orbitofrontal cortex and left hippocampus. They also showed a reduced RS functional connectivity (FC) of several regions of the frontoparietal and working memory networks, which may contribute to explain the cognitive deficits they showed at the neuropsychological assessment. Increased RS FC was detected at the level of the left insula and cingulum of the salience network. Fahr's and migraine patients experienced common increased RS FC of the left fusiform gyrus of the visual network.
CONCLUSIONS: RS FC abnormalities in the majority of RS networks occur in patients with Fahr's disease. Fahr's and migraine patients experienced common RS FC abnormalities of the visual network, which may contribute to clarify the presence of MWA in our two Fahr's patients.
