MRI Changes in Patients with Refractory Status Epilepticus Treated with Inhaled Anesthetics: A Case-Control Study

BACKGROUND: RSE carries a high morbidity and mortality rate. Most experts recommend aggressive therapy including continuous intravenous infusions or inhaled anesthetics. Reversible MRI changes suggestive of neurotoxicity have been attributed to high concentrations of isoflurane used for RSE but similar patterns have been seen from RSE itself.
DESIGN/METHODS: We performed a case-control study of 21 age- and sex-matched patients with RSE, 10 treated with isoflurane and 11 treated with intravenous anticonvulsants alone who underwent MR imaging. We reviewed their MRI reports and determined the frequency of reported abnormalities in the dura and leptomeninges, cortex, subcortical white matter, basal ganglia, thalamus, hippocampus, brainstem and cerebellum.
RESULTS: Any abnormality was identified in 9 (90.0%) cases and 8 (72.7%) controls (p = 0.59). Hippocampal abnormalities were reported in 5 (50.0%) cases and 1 (9.1%) control (p = 0.06). The frequency of abnormalities in all other areas was not significantly different between groups (p values = 0.31-1.0). Hippocampal abnormalities were present in all 3 (100%) patients with NORSE, all in the isoflurane group.
CONCLUSIONS: Our results do not support any extra-hippocampal neurotoxic potential of isoflurane when used in RSE. The frequency of hippocampal abnormalities in our cases may be the consequence of an independent and previously reported association with the NORSE syndrome, but an independent association with the use of isoflurane cannot be excluded on the basis of this data alone.
