Title Microcystic Macular Edema Is Associated with Optic Neuritis Independently of Multiple Sclerosis

Objective

Background BACKGROUND: MME were detected in a small subset of MS patients using optical coherence tomography (OCT). Associated with disease severity and higher in frequency in eyes with a previous optic neuritis (ON), the cause and relevance of MME are currently unknown.

Design/Methods DESIGN/METHODS: We retrospectively analyzed 216 MS patients, 39 patients with clinically isolated syndrome (CIS), 20 neuromyelitis optica spectrum disorder patients (NMOSD), 9 patients with chronic inflammatory optic neuropathy (CRION) and 121 healthy subjects (HC). While detecting MME on OCT B-scans we further investigated scanning laser ophthalmoscope images from the same subjects for characteristic changes of the ocular fundus and quantified the affected area.

Results RESULTS: MME were detectable in OCT of 22 eyes from 15 patients (5.3%) including 7 patients with bilateral MME. 21 of these eyes had a prior ON (95%). MME eyes showed a low visual acuity (VA; mean ± standard deviation (SD): 0.31 ± 0.38) and a long interval since the last ON (mean ± SD: 100 ± 100 months). All 22 affected eyes showed dark dotted texture changes in SLO images in form of a crescent around the fovea and were correctly classified on SLO images (sensitivity = 100%) with a high specificity (95.2%). The area of these fundus affections in MME eyes was correlated with VA and the time between ON and OCT examination.

Conclusions CONCLUSIONS: MME occur in various neuroinflammatory disorders associated with ON including MS and are strongly linked to clinical ON events. Furthermore, MME are not only detectable by OCT B scans but can be quantified using scanning laser ophthalmoscopy images provided by the same device, potentially easing MME detection and monitoring in daily routine.
