Cognitive Performance and Brain Integrity in Multiple Sclerosis: A Diffusion Tensor Imaging Study

BACKGROUND: Disconnection of white matter (WM) tracts and grey matter (GM) damage could be implicated in cognitive dysfunction in MS.
DESIGN/METHODS: We included 67 MS patients and 37 controls. Cognition was evaluated in patients by the Rao battery (normalizing through z-scores). The MRI included structural T1-MPRAGE and diffusion tensor imaging (DTI). Tract-based spatial statistics (TBSS) and voxel-based analysis (VBA) were used to compare DTI values between patients and controls and to localize regions where DTI fractional anisotropy (FA) or mean diffusivity (MD) levels correlated with cognitive z-scores in normal-appearing WM (NAWM) and GM in MS. We correlated z-scores with lesion likelihood through lesion probability mapping (LPM).
RESULTS: MS patients showed widespread reduced NAWM FA and increased GM MD in several regions. Verbal memory correlated with specific tracts and regions for each subtest in MS patients: the selective reminding test (SRT) long-term storage correlated with FA in corpus callosum (CC), corona radiata, posterior thalamic radiaton, sagittal stratrum and cingulated gyrus and with GM MD in left angular, occipital, lingual, fusiform and temporal cortex. SRT long-term retrieval displayed similar findings in NAWM, while correlation with GM MD was more extensive (i.e.hippocampus, left insula, angular, occipital and temporal cortex). SRT delayed recall (DR) correlated with FA in CC and with MD in left calcarine, lingual, hippocampus, thalamus and fusiform cortex. Visual memory DR correlated with FA in CC and posterior thalamic radiations. The Symbol Digit Modalities Test showed widespread correlation with NAWM FA. LPM showed periventricular and corona radiata areas correlating with SRT long-term storage and DR.
CONCLUSIONS: Both GM and WM microstructure have an influence on cognition that differs in different domains. We provide anatomic rendition of pathways implicated in the different processes of declarative verbal memory.
