Title Automated Analysis of Retinal Photographs: Results in Subjects with Hypertensive Retinopathy and CADASIL

Objective

Background Because of similarities between retinal and cerebral vessels, fundoscopy has been pointed toward as a potential window into the brain microcirculation. Full exploitation of fundoscopy in clinical settings, however, is limited, because currently available tools for assessment of retinal vessels are qualitative or heavily operator-dependent.

Design/Methods Retinal photographs were collected from subjects allocated to four groups: subjects with hypertensive retinopathy (n=16), subjects with CADASIL (n=11), and the respective age and gender matched controls (n=16 and 11). Mean-D (a parameter of fractal analysis), arteriole-venule diameter ratio, and Tortuosity Index of the retinal vessels have been carried out by means of two custom software plugins. Analysis of matched case-control data was performed using paired non-parametric mean comparison. Intraclass correlation was estimated as a measure of inter-rater reliability.

Results We found altered parameters in the hypertensive or CADASIL groups, as compared to the matched control subjects. High inter-rater reliability was observed for all the measurements.

Conclusions This method is a simple, almost operator-independent, and highly reproducible way of discriminating pathological conditions characterized by morphological changes of retinal vessels. The advantages include the reduction of analysis time and the minimized operator intervention. The novelty of our approach may improve confidence in fundoscopy as a tool to be exploited in clinical settings or trials.
