Title Weights and Volumetric Measures: The Effect of Weight Class on Brain Volume in Professional Combatants

Objective

Background Increasing exposure to repeated head trauma has been associated with lower volumes in various subcortical structures as measured by magnetic resonance imaging (MRI) in a cohort of professional fighters. However, little is known about other factors that may modify risk of brain injury from cumulative head trauma. Body weight, and thus frequency and force of punches, may influence the type of exposure a fighter may experience.

Design/Methods This is a cross sectional analysis of first year data on subjects participating in the Professional Fighters Brain Health Study, a longitudinal cohort study of professional boxers and mixed martial arts fighters. Subjects undergo MRI imaging on a yearly basis, with brain volumes obtained by automated methods. Weight class (divided into: < 140, 140-168, 168-200, >200 lbs), fight exposure, and their interaction were independent variables, with brain volumes as dependent variables. Age, education, race, and intracranial volume were included in all models and repeated measures ANOVA performed.

Results 155 male fighters were included for analysis. There was a marginally significant interaction between the number of professional fights and weight class (p=0.051). Decline in caudate volume as number of pro fights increases is moderated by weight class, with lower weight classes showing larger decreases than for higher weight classes. There were no significant findings with weight class and thalamic, putamenal or hippocampal volumes.

Conclusions Weight class may have a marginal influence on the relationship between exposure to head trauma and caudate volume in professional combatants.
