This study investigates incidence and predictors of increased risk of CVT among hospitalized pregnant patients in United States.
Cerebral venous thrombosis (CVT) in pregnancy is often reported in the last trimester or shortly after delivery however, the incidence among pregnant women is not really known.  In addition,  information about predictors of increase risk of CVT among hospitalized pregnant patients is scarce.  

We performed a cross-sectional analysis using the Nationwide Inpatient Sample (NIS), 2012–2015, of US adult hospitalizations with pregnancy. Overall incidence of CVT was calculated using NIS weighting. We identified our CVT patient subset from using codes (ICD-9-CM Diagnosis codes: 4376, 325 and 6715) from the International Classification of Diseases, 9th edition. Potential factors associated with increased risk of CVT in pregnancy were assessed using logistic regression.

Of 11,672,729 pregnant hospitalized patients, 1045 (0.01%) developed CVT during the index hospitalization. Important predictors of increased risk of CVT in pregnancy are: Age above 30 years (OR: 1.52, CI: 1.16-2.02 , p <.0001), coagulopathy (OR: 7.45, CI: 5.00-11.09 , p <.0001) and obesity (OR: 1.45, CI:1.16-2.02 , p= 0.0714). 

Older age, coagulopathy and obesity are important predictors of increased risk of CVT in pregnant women. Availability of aggressive treatment has not modified significantly mortality which requires further study.
