A Simple Question to Self-Report Myasthenia Gravis Severity Can Predict Patient Acceptable States

To determine how the patient acceptable symptom state (PASS) question is associated with a simple question (SQ) that ask patients to self-rate the status of myasthenia gravis (MG).
The SQ is a simple and validated question on what per cent of normal, patients feel with respect to their MG status (0 to 100), with higher scores indicating less MG-related symptoms. The dichotomous PASS question asks whether patients are content to remain in their current state over the next few months; thus assessing if patients feel well considering their overall MG symptoms. The SQ and PASS assess different constructs in MG patients, and we examined how they are associated.
We performed a retrospective chart review of consecutive MG patients and extracted the SQ and PASS responses, demographic, clinical and serological characteristics of patients and disease severity based on the myasthenia gravis impairment index (MGII).

Of 53 MG patients, 17 (32%) answered “No” to the PASS question, indicating  unsatisfactory health status. Patients who answered “No” had worse MGII scores than those who answered “Yes” (31.4 +/- 18.2 and 8.2+/- 10.8 , p <0.0001). We found a strong correlation between PASS response and SQ (r -0.692, p 0.000). The ROC curve had AUC of 0.907 (0.820-0.994, 95% CI) with a SQ cut-off of 70% to predict PASS response. Worsening of SQ percentage from last visit also correlated with PASS status (r:0.397, P 0.044).

 

A cut-point of 70%  in the SQ can be used to predict acceptable states (PASS) in patients with MG, whereby scores lower than 70% predict unsatisfactory MG status.
