Gray Matter Volume and Myelination Assessed by Magnetization Transfer Ratio Predict Disease Progression in Secondary Progressive Multiple Sclerosis: Exploratory Analyses from the EXPAND Study

To assess the predictive value of baseline magnetic resonance imaging (MRI) and magnetization transfer ratio (MTR, a measure of myelination) parameters on time-to-6-month confirmed disability progression on the Expanded Disability Status Scale (6mCDPEDSS) and a composite endpoint (EDSS and Symbol Digit Modalities Test; 6mCDPEDSS/SDMT) in the EXPAND Core and Extension studies.
In the EXPAND-Core, siponimod showed efficacy on MRI-lesion activity, total brain and gray matter (GM) atrophy, as well as on MTR measures in SPMS patients.
This exploratory analysis was restricted to patients randomized to siponimod (N=1099; MTR subset [n=402]) to avoid potential confounding effects due to variable treatment exposure and switching (placebo-siponimod) during EXPAND-Core and Extension. Patients were categorized into quartiles of baseline total normalized brain volume (NBV), cortical GM (cGM) and thalamic volume, T1-hypointense lesion volume (T1HLV), T2-lesion volume [T2LV], median normalized MTR of normal-appearing brain tissue (nMTR-NABT), normal-appearing white matter (nMTR-NAWM) and cGM (nMTR-cGM), and by absence/presence of gadolinium-enhancing lesions. Association with 6mCDPEDSS or composite 6mCDPEDSS /6-month confirmed meaningful worsening in SDMT (≥4-points) were analyzed using a Cox-regression analysis with the respective outcome as a baseline covariate comparing worst-quartile (WQ) versus best-quartile (BQ); p-values were not adjusted.
In EXPAND-Core, patients in the WQ (vs BQ) of nMTR-cGM and cGM volume had a higher risk of 6mCDPEDSS (hazard ratio [HRWQ/BQ] nMTR-cGM: 1.93, p=0.029; cGM: 1.58, p=0.022); non-significant trends were observed for nMTR-NABT and nMTR-NAWM (HRWQ/BQ: 1.38). For the composite 6mCDPEDSS/SDMT, cGM and thalamic volumes were predictive in both Core and Core+Extension (HRWQ/BQ cGM: Core 1.76, p=0.0009/Core+Extension 1.30, p=0.046; thalamic: Core 1.64, p=0.007/Core+Extension 1.66, p=0.0003). T2LV was predictive in Core+Extension (HRWQ/BQ: 1.32; p=0.041). No significant associations were observed for NBV, T1HLV and gadolinium-enhancing lesions.
The observed predictive values of GM volume and MTR support the potential clinical prognostic value of these parameters for disease progression in siponimod-treated SPMS patients.
