Long Term Safety Outcomes with Inebilizumab Treatment in NMOSD: the N-MOmentum Trial

To evaluate the long-term safety of inebilizumab in neuromyelitis optica spectrum disorder (NMOSD).

Rates of treatment-emergent (TE) adverse events (AEs), serious AEs (SAEs) and infusion-related reactions (IRRs) were comparable between the inebilizumab and placebo groups during the 28-week randomized controlled period of the N-MOmentum trial. Participants could enter the open-label period (OLP) for a minimum of 2 years, where the long-term efficacy and safety of inebilizumab was assessed.
OLP participants received intravenous inebilizumab 300 mg every 28 weeks. A limited interim analysis was performed, with a full OLP analysis planned at the end of the study (EoS; due Q4 2020). Owing to the expected volume of EoS data, only safety data will be presented here, including TEAE/TESAE profiles, severe and opportunistic infections, IRRs, anti-drug-antibodies, immunoglobulin levels, leucocyte counts and B-cell counts; efficacy will be covered in a companion abstract.
Interim analyses showed that 51/56 (91.1%) of those randomized to placebo (RP) and 165/174 (94.8%) of those randomized to inebilizumab (RI) entered the OLP. Mean (SD) OLP inebilizumab exposure in RP and RI groups was 2.4 (1.1) and 2.3 (1.1) years, respectively. TEAE incidence rates per 100 person-years were 304.5 (RP) and 251.4 (RI), most commonly urinary tract infection (UTI: 14.9 and 7.2), nasopharyngitis (7.1 and 6.9) and upper respiratory tract infection (4.7 and 5.7), respectively. Similarly, the most common TESAEs were UTI (4.7 and 0.5) and pneumonia (0.8 and 0.7), respectively. IRRs occurred in 6 (11.8%) RP- and 9 (5.5%) RI-participants. Two participants in the OLP died: one from complications of a severe NMOSD attack and the other from a CNS event of unclear etiology. No other deaths occurred. The full safety profile of longer-term inebilizumab treatment will be presented.

Interim analyses of the OLP indicate no additional safety concerns arising with multiple doses of inebilizumab in participants with NMOSD.
