A Functional Composite Endpoint to Characterize Disease Progression in Patients with Active or Non-active Secondary Progressive Multiple Sclerosis

To characterize disease progression using novel composite endpoints (CEPs) relevant to secondary progressive multiple sclerosis (SPMS) and evaluate their performance in active and non-active SPMS.
CEP captures disease progression more comprehensively as it accounts for functions less well or not captured by Expanded Disability Status Scale (EDSS) alone. A previous analysis, combining SDMT and EDSS, demonstrated high sensitivity in determining treatment effects. The current analysis additionally included 9-Hole Peg Test (9HPT) and Timed 25-Foot Walk test (T25FWT). 
In this post hoc analysis, two definitions for time to 6-month confirmed worsening (6mCW) were applied for all SPMS patients participating in the EXPAND Core study and in subgroups with active and non-active disease: CEP1 based on EDSS (1-point/0.5-point worsening from baseline of ≤5/>5, respectively), ≥4-points worsening on SDMT, and 20% increase in 9HPT; and CEP2 that in addition to CEP1 included the component of 20% increase in T25FWT (only for patients with baseline EDSS ≤5.5, since T25FWT was unstable in patients with higher baseline EDSS in the EXPAND study).
As previously reported, siponimod treatment reduced the 6mCW risk based on EDSS alone (HR [95% CI] by 26% (0.74 [0.60; 0.92]; p=0.006) in the overall population, 37% (0.64 [0.47; 0.87]; p=0.004) in the active group, and 13% (0.87 [0.64; 1.19]; p=0.376) in the non-active group. Using CEP1, risk reductions were 27% (0.73 [0.62; 0.86]; p<0.001), 30% (0.70 [0.55; 0.88]; p=0.003), and 21% (0.80 [0.63; 1.010]; p=0.061), respectively. Corresponding risk reductions with CEP2 were 25% (0.75 [0.64; 0.88]; p<0.001); 29% (0.71 [0.57; 0.89]; p=0.003); and 19% (0.81 [0.64; 1.02]; p=0.070).
Adding SDMT and 9HPT to the EDSS assessment (CEP1) allows the detection of treatment effects on a broader spectrum of symptoms in SPMS compared with EDSS, including in patients with non-active disease. Addition of T25FWT in CEP2 did not increase the compound measure’s efficiency.
