Quality Improvement Project for Improving Adherence to ICH Severity Documentation Using RapidAI

To establish interventions that address barriers to improve adherence to ICH severity documentation

The Joint Commission requires initial documentation of ICH score which helps stratify baseline severity of ICH and is a key performance metric for a comprehensive stroke center. The initial phase of our QI project found that at our institution the main barrier was a lack of knowledge of ICH score assessment, mainly due to the lack of availability of ICH volume data on radiology reports. 

We implemented the following interventions (1) Forming a multidisciplinary stroke committee and identifying providers involved and holding them accountable for ICH severity documentation (2) Incorporating Rapid Hyperdensity assisted ICH volume assessment to Epic, PACs and Stroke workflow (3) Provider education regarding the nuances of ICH severity discussions and documentation, (4) Reminder emails, messages (5) Monthly Multidisciplinary stroke committee meetings to identify and address barriers, reinforce and monitor progress.

Use of Rapid AI driven quality improvement initiative resulted in ICH severity documentation increasing from 0% to 100% in 10 months. Increased ICH severity documentation compliance and other improved quality metrics and performance measures led us to achieve the status of a Comprehensive stroke center.

Our AI-assisted QI project led to improvement in ICH severity documentation, which led our hospital to achieve the status of a Comprehensive Stroke Center. Using AI to improve stroke care is a major area for improvement.
