Title Safety and Tolerability of BG-12 (Dimethyl Fumarate) in Patients with Relapsing-Remitting Multiple Sclerosis: An Integrated Analysis of Phase 2 and 3 Placebo-Controlled Studies

Objective

Background Oral dimethyl fumarate has demonstrated positive efficacy in RRMS.

Design/Methods Eligibility criteria included age 18-55 years, RRMS diagnosis, and Expanded Disability Status Scale score 0-5.0. In Phase 3 studies, patients were randomized to dimethyl fumarate 240 mg twice (BID) or three times daily (TID) or placebo. One study also had a reference comparator arm (not presented here). The Phase 2 study included patients treated with dimethyl fumarate 240 mg TID and placebo. Safety and tolerability assessments included adverse event (AE) monitoring and laboratory tests.

Results A total of 2,428 patients (safety population) received placebo (n=836), or dimethyl fumarate BID (n=769), or TID (n=823). Overall, the incidence of AEs (92%, 95%, 93%), serious AEs (21%, 18%, 15%), serious infection (1%, 2%, 2%), malignancies (<1%, <1%, <1%), and discontinuations due to AEs (11%, 14%, 14%) were comparable in the placebo, BID, and TID groups, respectively. AEs with incidence ?5% higher in either dimethyl fumarate group versus placebo were flushing and related events (hot flush, erythema), gastrointestinal events (diarrhea, nausea, upper abdominal pain, abdominal pain), and rash. No opportunistic infections were reported in dimethyl fumarate groups. Overall incidences of hepatic and renal events were similar across groups. In dimethyl fumarate groups, mean lymphocyte counts decreased [sim]25-30% over the first year, then plateaued (remaining within normal limits). Four deaths in total were reported in dimethyl fumarate and placebo groups (none assessed as treatment-related).

Conclusions Results from the integrated analysis demonstrate an acceptable safety profile for dimethyl fumarate. Together with efficacy data from these studies, the results suggest that dimethyl fumarate has the potential to become a valuable oral treatment option for patients with RRMS.
