Etiologic Stroke Subtypes in the NINDS Stroke Genetics Network Study

Reproducible data on stroke subtypes in large multicenter datasets based on well-referenced and evidence-based criteria are scarce. We sought to determine stroke subtypes in a large international consortium of ischemic stroke studies which aims to identify the genetic basis of ischemic stroke subtypes.
We used the Causative Classification of Stroke System (CCS) to identify etiologic stroke subtypes. CCS is a web-based, automated, and evidence-based algorithm that identifies the most likely stroke etiology by harmonizing clinical stroke features and data from the stroke work-up. Adjudicators at each participating site trained and certified in CCS identified etiologic subtypes based on chart reviews. Ten percent of all adjudicated cases were readjudicated by members of the Phenotype Core or independent site adjudicators.
CCS subtypes were determined in 15,720 cases from 11 US and 11 European sites. There was a good agreement between adjudicators and readjudicators (kappa = 0.76, 95% CI: 0.73-0.79). The most common stroke subtype was cardio-aortic embolism (31%), followed by large artery atherosclerosis (17%), small artery occlusion (11%), undetermined causes (12%), and other uncommon causes (6%). A subtype could not be assigned in 23% of the cases because of insufficient evaluation. The overall distribution of etiologic categories varied with respect to age (<50 vs. ?50 years), sex, race, study site, cohort type (hospital-based vs. population-based), and brain imaging used (CT vs. MRI) (p<0.001 for all variables).
Ischemic stroke subtypes could be determined with high reliability in collaborative studies that include investigators with different expertise and background, institutions with different stroke evaluation protocols and geographic location, and patient populations with different epidemiological characteristics.
