Title Natalizumab Effects during a 6-Month Dose Interruption: Relationship of Pharmacokinetic (PK), Pharmacodynamic (PD), and MRI Measurements

Objective

Background Monthly infusions of natalizumab are a highly effective treatment for multiple sclerosis (MS). MS disease activity returns after discontinuation of natalizumab, although there are varied accounts of how long after discontinuation activity returns.

Design/Methods One hundred seventy-five MS patients treated with natalizumab for at least 1 year were enrolled. Patients were randomized to continue natalizumab, receive placebo [P], or start alternative immunomodulatory therapy (interferon beta-1a [IFN], glatiramer acetate [GA], or methylprednisolone [MP]) for 6 months. Monthly MRIs were conducted, along with measurements of natalizumab concentrations, alpha4-integrin saturation, lymphocyte counts, soluble VCAM (sVCAM), CD49d expression, and lymphocyte subsets.

Results All biomarkers for natalizumab were consistent with its expected effects. By 4 months after the last dose of natalizumab, lymphocyte counts, alpha4-integrin saturation, sVCAM, and CD49d expression from the groups not randomized to natalizumab returned to values consistent with those for untreated patients. No natalizumab activity was present in 77% of patients by 4 months after the last dose. On an individual patient basis, gadolinium-enhancing (Gd+) lesions did not appear until PD markers returned to levels found in untreated patients; the earliest lesion was reported 3 months after the last natalizumab dose. There was a relationship between alpha4-integrin saturation measured 4 weeks prior to lesions and Gd+ lesion appearance; an increased risk of Gd+ lesions was present when saturation was <70%. Trough (week 4) alpha4-integrin saturation was not predictive of Gd+ lesion occurrence.

Conclusions After natalizumab discontinuation, time to return of alpha4-integrin saturation to values found in untreated patients may vary across individuals. Alpha-4 integrin saturation is predictive of the risk of Gd+ lesion occurrence.
