Title Tolerability and Safety of Plasma Exchange Therapy in Patients with Myasthenia Gravis

Objective

Background It has been shown that PLEX is an effective treatment and comparable to IVIG in patients with moderate-severe MG. However concerns about the feasibility and tolerability of PLEX may lead physicians to avoid this effective therapeutic option.

Design/Methods In a randomized, placebo-controlled, masked-evaluator comparison study of IVIG with PLEX in patients with moderate to severe MG, 42 patients received up to 5 PLEX sessions. Demographic data, clinical responses, side effects of PLEX and technical details of PLEX administration were collected prospectively.

Results 40/42 patients completed PLEX treatment; 90% in an outpatient setting. 53% responded to treatment. 55% of patients did not experience complications. 45% had only mild-moderate side effects, the majority being citrate reactions and peripheral vascular issues that did not require termination of PLEX. In 83% of the patients, PLEX was performed via peripheral venous access. Two patients had severe adverse effects, with one being possibly related to PLEX. Co-morbid disease and age did not predict side effects.

Conclusions Our results support the use of PLEX as a safe and well-tolerated therapeutic option in treating patients with moderate-severe MG.
