Association of Triglyceride: High Density Lipoprotein Ratio with Intracranial Atherosclerotic Disease

Emerging data suggest that a triglyceride high density lipoprotein (TG):HDL-C ratio may be a better predictor of vascular risk than the traditional lipid measures such as Total cholesterol and LDL-C.
We identified patients with angiographically diagnosed symptomatic intracranial atherosclerotic disease (ICAD) from a prospectively maintained registry supplemented by retrospective chart review between 2007 to 2010 (n=40). Patient's lipid profile performed within 3 months of diagnosis was analyzed. The ICAD cohort was matched by age and gender to a normal population (without any vascular events including transient ischemic attack, stroke or death due cardiovascular event) from the Multi-Ethnic Study of Atherosclerosis (MESA) database. The MESA which is a population-based sample of 6,800 men and women aged 45-84 years, prospectively monitored with 3 cross sectional follow up of normal individuals in six communities in the United States.
Case control continuous variable analysis showed significantly higher TG/HDL ratio in ICAD subjects as compared to Control subjects (4.81 卤 3.11 vs. 2.95 卤 1.95 p=0.0002). In a logistic regression model, higher TG/HDL ratio was directly associated with ICAD (odds ratio [OR] 1.36, 95% confidence interval [CI] 1.13-1.65 p= 0.001). After adjusting for diabetes, hypertension, smoking, cigarette smoking and alcohol use higher TG/HDL ratio was directly associated with ICAD (OR 1.26, 95% CI 1.0-1.58 p= 0.048).
The serum TG: HDL-C ratio is associated with symptomatic ICAD. Further studies are needed to validate the use of TG: HDL-C ratio in risk stratification for population based screening and intervention.
