Sleep Disturbance in Patients with Parkinson Disease Presenting with Leg Motor Restlessness

BACKGROUND: The urge to move (UM), which is accompanied by an unpleasant sensation in the affected limbs, is a core symptom of restless leg syndrome (RLS). Patients with UM, though not fulfilling the essential RLS criteria, were diagnosed with LMR. A recent study showed that in early PD, LMR and not RLS occurs with a nearly 3-fold higher risk in UM patients than in controls (Neurology 2011;77:1941-1946). However, effects of LMR on sleep remain undetermined.
DESIGN/METHODS: Using questionnaires, we assessed 158 Japanese PD patients (male:female=65:93, 70.3卤9.4 years) for LMR, sleep time, and insomnia including sleep-onset insomnia and arousal during sleep. RLS was diagnosed according to the essential diagnostic criteria. We also assessed excessive of daytime sleepiness (EDS) with the Epworth Sleepiness Scale.
RESULTS: Forty-eight patients had UM; 18 (11%) and 30 patients (19%) were diagnosed with RLS and LMR, respectively. Sleep time of the RLS group, LMR group, and non-UM group were 5.8卤1.8 h, 6.5卤1.7 h, and 7.0卤1.5 h, respectively. Sleep time of RLS group was shorter than that of non-UM group (P=0.009). Frequencies of insomnia of the RLS, LMR, and non-UM groups were 77.7%, 63.3%, and 40.9%, respectively; frequencies of insomnia of the RLS and LMR groups were higher than that of non-UM group, respectively (P=0.004 and 0.029). Frequencies of EDS of the RLS, LMR, and non-UM groups were 61.1%, 46.7%, and 34.5%, respectively, and frequency of EDS of RLS group was higher than that of non-UM group (P=0.032).
CONCLUSIONS: Sleep time, frequencies of insomnia, and EDS of LMR group were intermediate between those of RLS and non-UM groups. LMR might be a novel contributing factor to sleep disturbance in PD patients.
