Relationship between Fatigue and Memory Performances in Multiple Sclerosis

BACKGROUND: Fatigue and memory impairments are common symptoms in multiple sclerosis (MS). Mood, cognition and fatigue interact. This can contribute to making a complaint misrepresentative of the objective evaluation.
DESIGN/METHODS: Our prospective study included 50 MS patients with complaint of fatigue. Subjective assessment of fatigue, memory function and mood was measured using the French version of the Fatigue Impact Scale (FIS), the self-assessment questionnaire memory (QAM) and the Beck Depression Inventory (BDI-II). Objective cognitive and memory performances were assessed using a French battery of neuropsychological tests specifically adapted to MS patients, the BCcog-SEP, including the Selective Reminding Test (SRT) testing the verbal episodic memory. Taking into account the confounding factors, we studied the correlations between subjective fatigue, memory complaint and performances in verbal episodic memory in our population.
RESULTS: BDI-II score, psychotropic and/or antiepileptic drugs use, EDSS score and form of MS were confounding factors and influenced at least one of the parameters studied among subjective fatigue, memory complaint and performances in verbal episodic memory. After adjustment, fatigue and memory complaint were not correlated with performances in SRT (p>0.05). Subjective fatigue was significantly associated with memory complaints (p<0.0001).
CONCLUSIONS: Fatigue complaint in MS should lead to assess thymic status and to look for side effects of psychotropic drugs. Although complaint of fatigue in MS was correlated with memory complaint, subjective fatigue was not the expression of memory impairment.
