Smartwatch Device for Tremor Accelerometry

BACKGROUND: Tremor caused by Parkinson disease (PD) and essential tremor (ET) can be difficult to differentiate. Accelerometry can measure characteristics of tremor, but basic frequency peak analysis cannot distinguish PD from ET tremor. Advanced analyses of accelerometer signals (mean power at harmonics of the peak frequency, asymmetry of the autocorrelation decay) have been used for this purpose in small studies. Accelerometers are now widely available in smartphone and wristwatch devices, which may simplify their use for office diagnosis.
DESIGN/METHODS: Adult patients with a clinical diagnosis of PD (n=4) or ET (n=6) tremor were enrolled. A 25g analog accelerometer and a smartwatch device were affixed to the dorsum of the predominantly affected hand. Acceleration was measured in three positions for sixty seconds: with hands (a) resting on an armchair, (b) outstretched, (c) holding a cup. Healthy controls (n=6) were recorded mimicking postural tremor (volitional tremor). The peak tremor frequency recorded by the analog accelerometer and the smartwatch device were statistically compared. Additional recordings were made to verify the accuracy of acceleration magnitudes.
RESULTS: Peak frequency estimates from the smartwatch and analog accelerometer had near perfect statistical concordance (concordance correlation coefficient 0.9975, 95% confidence interval 0.9931 - 0.9991). Acceleration magnitudes from the smartwatch were accurate.
CONCLUSIONS: A portable smartwatch device can record accurate acceleration magnitudes, and in our patients with hand tremor due to PD or ET and normal controls producing volitional tremor, estimates tremor frequency concordantly with a standard analog accelerometer. Further enrolment is required to determine the utility of such devices in classifying tremor of PD and ET.
