Impact of Meditation on Default Mode Network & Hippocampus in Mild Cognitive Impairment: A Randomized Trial

Meditation has been associated with increased hippocampal density and default mode network (DMN) connectivity. While stress may be associated with development of cognitive impairment, little is known about the neurophysiological impact of meditation in this population.
14 adults 55-90 years with MCI were randomized to MBSR (8 weekly meditation/yoga classes) vs. usual care. We conducted resting state fMRI at baseline and 8 weeks and our primary outcomes were: 1) functional connectivity between hippocampus, medial prefrontal cortex (MPFC), and posterior cingulate (PCC), key regions of the DMN; 2) rate of hippocampal atrophy. The Alzheimer's Disease Assessment Scale, cognitive subscale (ADAS-cog) was a secondary outcome.
After the intervention, participants in the MBSR group had increased functional connectivity between the PCC and bilateral MPFC and left hippocampus compared to those in the control group. Those in the MBSR group had trends of less bilateral hippocampal atrophy than those in the control group (median hippocampal volume difference from baseline to follow-up in MBSR, -32 vs. -274 in control, p=0.07). No significant differences were detected for median change from baseline for MBSR vs. control in ADAS-cog (-0.5 vs. 0, p=0.46).
MBSR appears to be a feasible non-pharmacological intervention in adults with MCI that may have a positive impact on the regions of the brain most related to MCI and Alzheimer's disease. Further research is warranted.
