Objective

Background BACKGROUND: Careful history taking is critical for arriving at accurate epilepsy diagnoses. A structured questionnaire (DISCOVER: Diagnostic Interview for Seizure Classification Outside of Video-EEG Recording) for administration by non-experts could be reviewed by experts to confirm the seizure diagnosis and classification for patients entering clinical trials.

Design/Methods DESIGN/METHODS: A research assistant administered a structured interview form to patients (18+ yo) admitted to our EMU with a suspicion of focal epilepsy. A separate form was provided for each seizure type. Another form was designed for observers to complete. The forms were reviewed by 2 epileptologists independently to determine a seizure classification and the degree of diagnostic certainty. The EMU report was the gold standard for classification of seizure types.

Results RESULTS: There were 157 seizure types in 49 patients, with observers providing information for 81 (50.6%) of the types. The inter-rater reliability was measured by Cohen's kappa, which was 0.43 (moderate) when all seizure types were considered, and increased to 0.54 when observer forms were available. When raters were moderately confident or greater in their classification (26% of the forms), the kappa increased to 0.84 (near perfect). Of these, 11 types were recorded in the EMU, and 9 (81.8%) had been correctly classified by both raters based on patient form review.

Conclusions CONCLUSIONS: The DISCOVER questionnaire, administered by a non-expert in epilepsy, provided information to epileptologists which enabled confident classification of 26% of seizure types described. When available, EMU monitoring confirmed these as correct classifications for most of the seizures. A large portion of the other seizure types were either simple partial seizures or non-epileptic. This questionnaire may allow for the confirmation of appropriateness during patient selection for clinical trials in epilepsy.
