Etiological Prevalence and Mortality Predictors in Elderly with Epileptic Seizures Admitted to a Terciary Centre in Salvador/Bahia - Brazil

BACKGROUND: Epilepsy in the elderly has high incidence and prevalence. It is the most common, serious, neurologic condition, following cerebrovascular and neurodegenerative disorders. Immediate diagnosis is crucial to avoid brain injury and reduce case fatality rate. There are few reports focusing the prevalence of epileptic events in the elderly inpatients.
DESIGN/METHODS: Retrospective analysis was performed on hospitalized elderly individuals (? 60 years old) with epilepsy and epileptic seizures who were under assistance of the neurology team of Sao Rafael Hospital, in Salvador/Brazil, from 2009 to 2010.
RESULTS: One hundred and twenty patients were analyzed, aged 75 卤 9 years, 55% females. The most common etiology for all subjects was ischemic stroke (37%), followed by neoplasias (13%), hemorrhagic stroke (12%), dementia (11.7%) and metabolic disturbances (5.8%). The group was subdivided in two, using median age (75 years).The analysis of etiologies showed that ischemic stroke was predominant in younger subgroup (37% vs 30%), while dementia was in the older one (18.9% vs 13.7%), with no statistical significance (p=0.23). In a univariate analysis, disease duration (p=0.04), status epilepticus (p<0.001) and metabolic disorder (p=0.005) were associated with mortality. After adjustment by logistic regression, only status epilepticus remained an independent predictor of death (odds ratio = 13; 95% CI = 2.3 - 72; P = 0.004).
CONCLUSIONS: (1) Stroke and neoplasias are the most common causes of epilepsy and epileptic seizures in the elderly; (2) Status epilepticus independently predicts mortality during hospitalization.
