CONCLUSIONS: Discussion: This unusual symptom of 'tilting' of the patient's environment has previously been described involving both visual fields and has been termed 'environmental tilt'. It is thought to involve vestibular-otolith disconnection and has been localised to lesions of the cerebellum and / or lateral medulla. The perception of verticality is mediated by inputs from the vestibular, somatosensory and visual systems, indicating verticality in an independent but congruent manner. The unilateral nature of symptoms in this patient was perhaps due to the location of the multiterritory infarctions.
