Cyclophosphamide Does Not Affect Fertility in Young Multiple Sclerosis Patients

BACKGROUND: It is well known that Cyc can induce ovarian failure and infertility and can have teratogenic effect on the foetus. There have been few case report of successful pregnancy after Cyc treatment in women with autoimmune disease or malignancies, but there is no available information about fertility and pregnancy outcomes in MS patients previously exposed to Cyc administration.
DESIGN/METHODS: We reviewed retrospectively the medical records of MS patients in childbearing age consecutively referring to the MS center of Catania, who had been treated with intra venous (i.v.) Cyc. We found 11 patients who experienced a pregnancy after having received Cyc treatment.
RESULTS: Out of the 11 patients, two are still pregnant, one underwent a voluntary abortion, the remaining 8 had a successful delivery. The mean age was 26卤1.9 years at the beginning of Cyc treatment and 30.8卤1.9 years at the time of conception. Women received an average cumulative doses of Cyc of 16.1卤2.4 g. The time lapse between the last administration of Cyc and conception was 3.27卤1.9 years. Four women had a pre-term delivery. One patient experienced a spontaneous abortion before the successful pregnancy. One child was small for gestational age. The average baby body weight was 2964.4卤221.0 g. All babies achieved the developmental milestones (walking and speaking) within one year.
CONCLUSIONS: We describe 11 pregnancies in women with MS previously exposed to Cyc treatment: eight were successful, two are still on going and one experienced a voluntary abortion. Our findings could encourage clinicians to use Cyc for short period in selected cases of young MS patients of childbearing age.
