Observational Program Tysabri 24 Plus: Evaluating Efficacy and Safety of Natalizumab Therapy beyond Two Years

BACKGROUND: In AFFIRM and post-marketing studies, NAT revealed high efficacy. NAT pts may develop progressive multifocal leukoencephalopathy (PML). Risk factors are the exposition to JC virus, pretreatment with immunosuppressants (IS) and NAT therapy >2 yrs. Thus, the risk/benefit profile needs to be better understood.
DESIGN/METHODS: This German multicenter study prospectively documents clinical status including Demtect score, MRI findings, patient management and since 01/2012 optionally anti-JCV antibody status in five 3-monthly visits during 1 yr of follow up in RRMS patients with NAT therapy >= 2 yrs. The study started in July 2010 (recruitment goal of 1000 pts reached in Oct 2011).
RESULTS: In this interim analysis (Feb 20, 2012) 948/1014 pts had complete baseline (BL) documentations. The patient population studied consisted of 66.8% female patients with a mean age of 40.4 yrs, and a disease duration of 10.6 yrs. The annualized relapse rate (ARR) in the yr before NAT was 2.2, the EDSS 3.5.; the Demtect score at BL 14.9. 18.9% of the patients received at least one IS prior to NAT. So far, 393 pts were documented in the last follow-up visit. Their mean ARR since BL was 0.14, EDSS 3.5, and Demtect score 16.0. 8.0% of pts discontinued, 3.4% interrupted NAT therapy; serious adverse events were documented in 11 pts (including PML confirmed in 1 case, suspected in another).
CONCLUSIONS: This interim analysis confirms sustained efficacy of NAT in reducing clinical disease activity with a generally consistent known safety profile. A detailed analysis after completion of the study will be presented and may provide data for the optimization of patient selection, monitoring and risk stratification.
