BG-12 (Dimethyl Fumarate) Treatment for Relapsing-Remitting Multiple Sclerosis (RRMS) Increases the Proportion of Patients Free of Measured Clinical and Neuroradiologic Disease Activity in the Phase 3 Studies

BACKGROUND: In the 2-year Phase 3 CONFIRM and DEFINE studies, oral dimethyl fumarate significantly improved clinical and magnetic resonance imaging (MRI) outcomes of RRMS vs placebo.
DESIGN/METHODS: RRMS patients were randomized to oral dimethyl fumarate 240 mg twice (BID) or three times daily (TID), placebo, or subcutaneous glatiramer acetate (GA; reference comparator, CONFIRM only). Absence of clinical disease activity over 2 years was analyzed in the intent-to-treat (ITT population) as no relapses and no disability progression. Absence of neuroradiologic (no new/enlarging T2 and no gadolinium-enhancing lesions) and overall disease activity (no measured clinical or neuroradiologic disease activity) were analyzed in the MRI cohort.
RESULTS: The ITT population of CONFIRM and DEFINE comprised 1,417 and 1,234 patients (681 and 540 in the MRI cohort), respectively. In CONFIRM, the proportion of patients in dimethyl fumarate BID, TID and GA vs placebo groups free of clinical disease activity was 69%, 73% and 66% vs 57% (p=0.001, p<0.0001 and p=0.01 vs placebo). In the MRI cohort, corresponding values were 24%, 29% and 21% vs 11% (p=0.002, p=0.0005 and p=0.273) for neuroradiologic activity, and 18%, 20% and 12% vs 7%, respectively (p=0.002, p=0.003 and p=0.124) for overall (clinical and neuroradiologic) activity. Similar results were found in DEFINE, with 28% and 26% of patients in dimethyl fumarate BID and TID groups, vs 15% with placebo, free of overall measured disease activity (p=0.012 and 0.033, respectively).
CONCLUSIONS: Across both studies and cohorts, findings from these post-hoc analyses of the Phase 3 data in RRMS patients indicated significantly higher proportions of patients free of measured clinical, neuroradiologic, and overall disease activity with dimethyl fumarate than with placebo.
