Title Clinical Efficacy of BG-12 (Dimethyl Fumarate) for Relapsing-Remitting Multiple Sclerosis (RRMS) According to Prior Therapy: An Integrated Analysis of the Phase 3 DEFINE and CONFIRM Studies

Objective

Background BACKGROUND: Integrated analysis of DEFINE/CONFIRM was conducted to estimate more precisely the treatment effect of dimethyl fumarate versus placebo, contingent upon homogeneous baseline characteristics and treatment effects across studies.

Design/Methods DESIGN/METHODS: Eligible patients were randomized to oral placebo or dimethyl fumarate 240 mg twice (BID) or three times daily (TID), or subcutaneous GA (reference comparator; CONFIRM only). Primary endpoints at 2 years were the proportion of patients relapsed (DEFINE) and annualized relapse rate (ARR [CONFIRM]).

Results RESULTS: The integrated intent-to-treat (ITT) population comprised 771, 769 and 761 patients assigned to placebo, dimethyl fumarate BID and TID, of whom 221, 202 and 193 had received treatment with prior approved ABCR therapy(s). Among these patients, 164, 155 and 145 had received 1 prior ABCR, and 57, 47 and 48 had received 2 or more ABCR therapies, respectively. Dimethyl fumarate BID and TID showed a consistent effect on clinical outcomes over 2 years versus placebo. Rate ratios (and 95% confidence intervals) for reductions in ARR with dimethyl fumarate BID and TID versus placebo at 2 years were: any prior ABCR 0.60 (0.53, 0.82) and 0.48 (0.34, 0.68); 1 ABCR 0.60 (0.42, 0.85) and 0.41 (0.28, 0.61); and 2+ ABCR 0.57 (0.29, 1.13) and 0.74 (0.37, 1.45), respectively.

Conclusions CONCLUSIONS: The integrated analyses indicate consistent clinical efficacy of both doses of dimethyl fumarate in RRMS across patients previously treated with one or more ABCR therapies for MS.
