Standardizing Evaluation of First Time Nonfebrile Seizure in Children at Children's Hospital Boston Based on Evidenced Based Guidelines

BACKGROUND: While child neurologists frequently manage children with first time non-febrile seizures, knowledge and application of evidenced based guidelines for management of these children may be lacking. This study aims to assess adherence to 好色先生 (AAN) practice parameters on children with first non-febrile seizure amongst Boston Children's Hospital(BCH) child neurologists.
DESIGN/METHODS: 35 child neurologists at BCH were surveyed on knowledge based and case based questions related to the AAN Practice Parameters on children with first time non-febrile seizures. Results were analyzed using a Fisher's exact test.
RESULTS: 83% of the child neurologists were aware of the practice parameters. 43% of child neurologists followed all of the parameters in clinical practice and 50% followed some of the practice parameters. On knowledge based questions, all child neurologists scored highly (80-100% correct) on appropriate ordering of diagnostic toxicology screen, MRI brain, EEG, lumbar puncture, prolactin, and CK as suggested by the AAN guidelines. However, child neurologists scored low on the appropriate ordering of an emergent head CT scan (59% correct). Child neurologists also scored low on appropriate ordering of routine electrolyte laboratory tests (54% correct). On 3 case based questions, variability on appropriate imaging and laboratory tests was similarly seen, whereas all child neurologists ordered an EEG appropriately.
CONCLUSIONS: Child neurologists have knowledge of AAN practice parameters on children with first non-febrile seizure, but knowledge of appropriate neuroimaging and electrolyte tests based on AAN recommendations varies amongst physicians. This study highlights the need to translate practice guidelines of a first time non-febrile seizure in children into clinical practice in a more standardized algorithm.
