Prenatal and Perinatal Morbidity in Children with Tic Disorders. A Mainstream School-Based Population Study in Central Spain

BACKGROUND: Tic disorders are a childhood onset neurodevelopmental disorder. Whereas there is consistent literature documenting the association of prenatal and perinatal morbidity with attention deficit disorder (ADHD), little is known about this association with tic disorders.
DESIGN/METHODS: Nested case-control design from a mainstream school-based sample in central Spain. Tic disorders and comorbid neuropsychiatric disorders were diagnosed based on DSM-IV TR criteria. Morbidity related to pregnancy, maternal disturbances, delivery, fetal distress, and perinatal information was collected by medical chart retrospective review. Parametric tests were used for comparisons and logistic regression analysis to study the association of tics with exposure to pre-perinatal morbidity.
RESULTS: From a sample of 1867 schoolchildren screened for tic disorders, 526 were assessed by the neurologist, and pre-perinatal history was obtained in 226 children (154 males [68%], and 71 with tics [31%]). After adjusting for confounding variables including intelligence quotient, fetal distress, presence of ADHD, and maternal morbidity, cesarean section (OR=6.2, p=0.001), family history of tics (OR=5.1, p=0.03), and maternal smoking (OR=3.1, p=0.03) were highly associated with tic disorders.
CONCLUSIONS: Women with fetuses at risk for tic disorders should receive counseling to avoid smoking and cesarean section if possible, to minimize the odds of tic disorders.
