Title Characteristic Features of Myositis with Anti-Mitochondrial Antibodies

Objective

Background BACKGROUND: Anti-mitochondrial antibodies are autoantibodies detected in over 90% of primary biliary cirrhosis (PBC) cases. Recently, it is reported that myositis with anti-mitochondrial antibodies accounts for 10% of all inflammatory myopathy cases and that it represents one of characteristic subgroups of myositis.

Design/Methods DESIGN/METHODS: We compared clinical manifestations, histological features and response to therapy between 8 patients of inflammatory myopathies with anti-mitochondrial antibody-positive and 23 antibody-negative patients. A Mann-Whitney's U test or Fisher's exact probability test were used for comparing both groups.

Results RESULTS: Patients with anti-mitochondrial antibodies showed chronic clinical course significantly. Cardiac arrhythmia and left ventricular systolic dysfunction were significantly frequent. Muscles of the extremities are involved to a lesser extent than trunkal muscles. Neither the mean vital lung capacity or mean arterial carbon dioxide level in arterial blood was not significantly different between two groups, while antibody-positive patients tended to have a restrictive ventilator defect. Serum creatine kinase levels before treatment were relatively low in antibody-positive group without statistical significance. Histologically, lymphocytic infiltration in biopsied muscle was significantly fewer in patients with anti-mitochondrial antibodies compared to patients without these antibodies. Almost all patients with anti-mitochondrial antibodies presented good response to corticosteroid therapy.

Conclusions CONCLUSIONS: In patients with myositis, anti-mitochondrial antibodies may be a biomarker related to cardiac complications and characteristic distribution in affected muscles.
