Title Cerebral Reperfusion and Clinical Outcomes after Endovascular Treatment for Acute Ischemic Stroke

Objective

Background BACKGROUND: In a number of clinical trials, case series and comparative nonrandomized studies reported in the last decade, improvement over time of recanalization rate was not paralleled by a consistent amelioration of clinical outcomes. We hypothesized that, given case selection, these studies may not reflect real world variability of ischemic stroke.

Design/Methods DESIGN/METHODS: The Italian Registry of Endovascular Treatment in Acute Stroke (IRETAS) consecutively records patients treated with intra-arterial thrombolysis or trombectomy in 25 tertiary stroke centres . We studied successful reperfusion, defined as Thrombolysis in Cerebral Infarction (TICI) score 2b-3, as independent predictor of good functional outcome (3 months modified Rankin scale < 2), controlling for confounders. Safety measures included symptomatic intracranial hemorrhage (s-ICH), according to ECASS definition, peri-procedural adverse events and death.

Results RESULTS: From 2008 to 2012, 748 patients were registered. Outcome data were complete for 527 (70.5%) patients . Outcome predictors were similarly prevalent comparing patients who were included to those excluded from the analysis. Median age of the 527 patients was 68 (54-75) years; 56.5% were males. Baseline median NIHSS was 18 (13-22). s-ICH occurs in 51 (9.7%) patients, procedural adverse events in 25 (4.7%) patients. Complete reperfusion was achieved in 303 (57.5%) patients. Two hundred twenty five (42.7%) patients had a good functional outcome. Death rate was 24.9% After adjusting for age, gender, baseline NIHSS, time from onset to groin puncture, site of occlusion, treatment modality, successful reperfusion strongly predicted good functional outcome (OR 1.92, 95%CI, 1.60-2.31).

Conclusions CONCLUSIONS: In a real word practice scenario good functional outcome was strongly associated with successful reperfusion.
