Validation of the Falls Efficacy Scale-International (FES-I) as a Predictor of Falls in People with Multiple Sclerosis

BACKGROUND: PwMS fall frequently and fear falling. The Falls Efficacy Scale-International (FES-I) is a cross culturally validated questionnaire assessing fear of falling and perceived ability to avoid falls. The FES-I has not been validated in PwMS.
DESIGN/METHODS: Subjects were 58 PwMS age 18-50 with Expanded Disability Status Scale scores ? 6.0 participating in a study of mechanisms of imbalance in PwMS. Subjects completed the FES-I, a retrospective fall questionnaire and six monthly prospective fall calendars. Mean FES-I scores for fallers (? one fall) and non-fallers were compared with t-tests. Sensitivity, specificity and cut-off scores were determined by ROC curves.
RESULTS: 53 subjects completed the study and were included in this analysis. 27 and 40 subjects fell in the prior 2 and 12 months respectively; 21 and 37 fell in the following 2 and 6 months respectively. Mean FES-I scores were significantly different for fallers and non-fallers: prior 2 months (30.6 卤 10.7 versus 20.9 卤 4.6; p=0.0007); prior 12 months (27.5 卤 10.1 versus 21 卤 5.2; p=0.002); following 2 months (31.1 卤 10.2 versus 22.5 卤 7.5; p=0.002); following 6 months (27.8 卤 10.2 versus 21.4 卤 5.8; p=0.003). An FES-I score of 25 maximized sensitivity (76%) and specificity (78%) for predicting falls in the following 6 months. An FES-I score of 34 (sensitivity 92%, specificity 48%) is recommended when using this test to screen for risk of falls.
CONCLUSIONS: The FES-I discriminates between PwMS who have and have not fallen and, with a cut-off score of 34, provides excellent screening for fall risk over the following 2 and 6 months.
