Frequency of Infections during Treatment with Teriflunomide: Pooled Data from Three Placebo-Controlled Teriflunomide Studies

BACKGROUND: Teriflunomide is a novel, once-daily, oral immunomodulatory therapy recently approved in the US for relapsing multiple sclerosis (RMS). Teriflunomide limits expansion of stimulated T and B cells while appearing to preserve resting lymphocyte function.
DESIGN/METHODS: Safety data were pooled from three (1 phase 2, 2 phase 3) placebo-controlled teriflunomide trials. Patients with RMS received once-daily oral teriflunomide 14mg (n=786), teriflunomide 7mg (n=838), or placebo (n=806) for 36 weeks (NCT01487096), 108 weeks (NCT00134563, TEMSO), or 48-152 weeks (NCT00751881, TOWER).
RESULTS: The frequency of infections was similar between groups (14mg, 53.4%; 7mg, 54.2%; placebo, 54.5%). Relative risk (RR) of infection did not increase with teriflunomide 14mg (RR=0.98; 95% confidence interval [CI] 0.90,1.07) or 7mg (RR=0.99; 95% CI 0.91,1.09) versus placebo. Serious infections occurred infrequently and with similar incidence across groups (14mg, 2.5%; 7mg, 2.4%; placebo, 2.5%). Two fatal infections occurred: bacterial sepsis (14mg) and respiratory infection (placebo). Four serious opportunistic infections were reported: cytomegalovirus (CMV) hepatitis and gastrointestinal tuberculosis (14mg); herpes zoster and hepatitis C with CMV (placebo). No cases of progressive multifocal leukoencephalopathy occurred. Hematologic laboratory data showed decreases in mean WBC counts in teriflunomide treatment groups of [sim]15% or less, primarily of neutrophils and lymphocytes; these occurred during the first 6 weeks and remained stable during treatment. There was no evidence of a link between WBC decrease and occurrence of infections.
CONCLUSIONS: Teriflunomide had a low incidence of serious infections, which was not different from placebo. Mean reductions in WBC counts were small in magnitude, often stabilized during treatment, and were not associated with infections. These findings provide evidence that risk of infection is not increased by teriflunomide treatment.
