Relapse Outcomes with Alemtuzumab vs IFNB-1a in Active Relapsing-Remitting Multiple Sclerosis Patients Who Experienced Disease Activity While on Prior Therapy (CARE-MS II)

BACKGROUND: In CARE-MS II, alemtuzumab reduced the relapse rate over 2 years compared with SC IFNB-1a (0.26 vs. 0.52; 49% reduction; p<0.0001), in RRMS patients who relapsed on prior therapy.
DESIGN/METHODS: Active RRMS patients who had relapsed on prior therapy were randomized to receive alemtuzumab 12 mg intravenously on 5 days at study start and 3 days 1 year later or SC IFNB-1a 44 [micro]g 3-times weekly. Relapse events during the study were required to last ?48 h, required objective signs as assessed by blinded raters, and were adjudicated by an independent committee.
RESULTS: Over 2 years, the proportion of relapse-free patients was 65.4% (95% CI: 60.7, 69.7) with alemtuzumab and 46.7% (95% CI: 39.5, 53.5) with SC IFNB-1a representing a 47% reduction in the risk of relapse (p<0.0001). Number of patients needed to be treated with alemtuzumab instead of SC IFNB-1a to prevent 1 relapse during 2 years was 1.95 (95% CI: 1.28, 4.17). The effect of alemtuzumab on relapse rate was evident in the first year (0.28 vs. 0.59 for SC IFNB-1a; 54% reduction; p<0.0001) and was maintained for the duration of the study. Treatment effects favoring alemtuzumab were also seen in supportive analyses of the risk of severe relapses (48% reduction, p=0.012), steroid-treated relapses (56% reduction, p<0.0001), and relapses requiring hospitalization (55% reduction, p=0.0045). Efficacy of alemtuzumab was affirmed in a sensitivity analysis in which additional covariates were evaluated.
CONCLUSIONS: Alemtuzumab reduced relapses more effectively than high-dose, high-frequency SC IFNB-1a in active RRMS patients who relapsed on previous therapy. Superiority of alemtuzumab in reducing relapse risk is a robust finding based on sensitivity analyses.
