Title Detection, Incidence, and Management of Thyroid Autoimmunity in Comparison of Alemtuzumab and Rebif® in Multiple Sclerosis (CARE-MS) I and II

Objective

Background BACKGROUND: Alemtuzumab showed efficacy superior to subcutaneous interferon beta-1a (IFNB-1a) in CARE-MS I and II and increased risk of thyroid disease. In the phase 2 CAMMS223 study, 23% of patients experienced thyroid disorders through 36 months and 30% through median follow-up of 57 months; rate of thyroid disorders peaked the third year following initial alemtuzumab treatment and declined thereafter.

Design/Methods DESIGN/METHODS: CARE-MS comprised two 2-year phase 3 trials, in treatment-naive patients (CARE-MS I) and in patients who had disease activity on prior therapy (CARE-MS II). Patients received alemtuzumab 12mg on 5 days at entry and on 3 days 12 months later; fewer CARE-MS II patients received 24mg in an exploratory arm. Thyroid function testing was performed at baseline and quarterly thereafter.

Results RESULTS: At the end of the study, 1943 person-years of follow-up were available from 972 alemtuzumab-treated patients. Thyroid disorders were reported in 22 (5.7%) of IFNB-1a patients, none serious; and in 168 (17.3%) alemtuzumab patients, 8 (0.8%) serious. Further results for alemtuzumab patients: Rate of thyroid disorders for the 2-year study period was 0.13 per person year (0.007 for serious thyroid disorders). Hypothyroidism was the most frequently reported thyroid disorder, affecting 6.9% of patients (n=67), followed by hyperthyroidism (6.1%, n=59), abnormal thyroid laboratory investigations (5.7%, n=55), and goiter (1.6%, n=16). Most thyroid disorders were managed with conventional oral therapies; 3 patients had thyroidectomy and 2 received radioactive iodine ablation. Extension study data will also be reported.

Conclusions CONCLUSIONS: CARE-MS thyroid disorders occurred in a similar proportion of patients as in CAMMS223. The safety monitoring program in place allowed for early detection and effective management. These disorders tend to be delayed in nature so incidence is expected to rise with continued follow-up.
