Efficacy of Alemtuzumab vs IFNB-1a in Relapsing-Remitting Multiple Sclerosis Patients Who Experienced Disease Activity While on Prior Therapy (CARE-MS II): Subgroup Analysis by Previous Disease Modifying Therapy (DMT) Use

BACKGROUND: In CARE-MS II, alemtuzumab demonstrated a 49% reduction in relapses (rate ratio 0.51, p<0.0001) and 42% reduction in 6-month sustained accumulation of disability (SAD; p=0.0084) vs subcutaneous (SC) IFNB-1a over 2 years in RRMS patients who relapsed on prior therapy.
DESIGN/METHODS: Patients were randomized to alemtuzumab (12mg; IV on 5 days at study start and 3 days 12 months later) or IFNB-1a (44[micro]g SC 3[times] weekly). Subgroup analyses were conducted to assess influence of prior DMT therapy on relapse rate and SAD.
RESULTS: Of 628 patients randomized to SC IFNB-1a or alemtuzumab 12mg, 518 (82.5%) had prior IFNB (219 [34.9%] SC IFNB-1a; 166 [26.4%] IM IFNB-1a; 217 [34.6%] IFNB-1b) and 215 (34.2%) had glatiramer; all had previous exposure to ?1 or both of these. Alemtuzumab was more effective than IFNB-1a at reducing relapse rate among patients who had received prior SC IFNB-1a treatment (rate ratio 0.44; 95% CI:0.29-0.67) and those that had not (0.55; 0.40-0.76). Similar effects were observed in patients whether they had received any prior IFN (yes: 0.49; 0.37-0.64; no: 0.60; 0.32-1.10) or glatiramer (yes: 0.52; 0.34-0.78; no: 0.50; 0.36-0.69). Alemtuzumab reduced risk of SAD in patients grouped according to whether they had or had not received prior treatment with SC IFNB-1a (46% and 39% reduction, respectively), any prior IFN (42% and 43% reduction) or glatiramer (44% and 41% reduction). Similar effects of alemtuzumab over SC IFNB-1a were evident in patients using 1 or ?2 prior DMT on ARR rate ratio (1 DMT: 0.56; 0.40-0.77; ?2: 0.41; 0.28-0.61) and risk of SAD (45% and 39% reduction, respectively).
CONCLUSIONS: Efficacy of alemtuzumab was superior to IFNB-1a across subgroups stratified by type of prior therapy, duration of prior exposure and number of DMTs previously used.
