Objective

Background BACKGROUND: It has been recently described that the prevalence of neurodegenerative parkinsonism in small and isolated European communities might be higher than usually estimated in the general population. We have recently described this phenomenon in one of the villages (population 1500) of the specific region of south-eastern Moravia, where we have found the prevalence of 2.5% (in the population older than 50 years). The presented study involved the whole region, i.e., 10 villages (population of 7140 with approx. 2350 older than 50 years).

Design/Methods DESIGN/METHODS: Three-stage ascertainment method was used; in the first stage, each patient visiting GP for any reason over the study period of three months completed the screening questionnaire for parkinsonism. In the second stage, trained primary care neurologists identified all persons with positive signs of parkinsonism among those screened positive. In the third stage, all identified persons were admitted to the tertiary movement disorders center to confirm or exclude the diagnosis of parkinsonism.

Results RESULTS: The sample consisted of 1039 villagers; 193 persons were screened positive, and 69 of them were identified as manifesting signs of parkinsonism; the diagnosis was confirmed upon examination in the tertiary movement disorders center.

Conclusions CONCLUSIONS: The prevalence rate of 2.9% in the population older than 50 years substantially exceeds the estimated prevalence in the common Central-European population, and also the prevalence in recently published studies from the other parts of Europe; future research will focus on the heredity in the families in which has parkinsonism been identified.
