Teriflunomide Reduces Relapse-Related Sequelae, Hospitalizations and Corticosteroid Use: A Post-Hoc Analysis of the Phase 3 TOWER Study

BACKGROUND: Teriflunomide is a once-daily, oral, disease-modifying therapy recently approved in the US for the treatment of relapsing forms of multiple sclerosis. Teriflunomide (14mg and 7mg) significantly reduced annualized relapse rates by 36.3% (p=0.0001) and 22.3% (p=0.0183), respectively, versus placebo in TOWER. A similar post hoc analysis of the Phase III TEMSO study (NCT00134563) showed that teriflunomide reduced annualized rates of relapse with sequelae and relapses requiring healthcare resources.
DESIGN/METHODS: TOWER was a randomized, double-blind, placebo-controlled, parallel-group study (N=1169). The following outcomes were analyzed in the intent-to-treat population (N=1165): relapses with neurological sequelae (defined by increase in Expanded Disability Status Scale (EDSS)/Functional Score (FS) 30 days post-relapse [sequelae-EDSS/FS] or as determined by the investigator [sequelae-investigator]), relapses leading to hospitalization, relapses treated with corticosteroids, and number of nights in hospital for relapse. Annualized rates for each outcome were derived using a Poisson model with robust error variance, with treatment, baseline EDSS strata and region as covariates.
RESULTS: Compared with placebo, teriflunomide significantly reduced annualized rates of relapses with sequelae-EDSS/FS (14mg by 33.0% [p=0.0081]; 7mg by 26.4% [p=0.0315]) and sequelae-investigator (14mg only; by 53.5% [p=0.0004]), relapses leading to hospitalization (14mg only; by 33.6% [p=0.0155]) and relapses treated with corticosteroids (14mg by 35.7% [p=0.0002]; 7mg by 21.5% [p=0.0337]). In addition, patients treated with teriflunomide 14mg spent significantly fewer nights in hospital per relapse (1.7 vs 3.4 for placebo; p=0.0285).
CONCLUSIONS: Compared with placebo, both doses of teriflunomide significantly reduced the occurrence of relapses with sequelae-EDSS/FS and relapses treated with corticosteroids; teriflunomide 14mg also significantly reduced relapses with sequelae-investigator and relapses leading to hospitalization. As a result, teriflunomide may reduce relapse-related healthcare costs.
