Ischemic Stroke Compromises Various Aspects of Male Sexual Function

BACKGROUND: Ischemic stroke can deteriorate sexual function. Prevalence of male sexual dysfunction after stroke is not well determined.
DESIGN/METHODS: In 56 men, aged 64.39卤10.64 years, who had ischemic stroke within 24 months before evaluation, we assessed prevalence and severity of disorders in arousal ability, desire, erectile function, ejaculatory function, orgasmic function, and sexual satisfaction before and after stroke, using a self-designed questionnaire consisting of multiple visual analogue scale (VAS) questions (0=never; 10=always) (Wilcoxon-test; significance: p<0.05).
RESULTS: After stroke, VAS-values decreased significantly from pre-stroke VAS-values for arousal ability (4.61卤2.78 post-stroke vs. 6.37卤2.42 pre-stroke; p=0.0001), desire (4.42卤2.93 post-stroke vs. 6.37卤2.56 pre-stroke; p=0.0001), ejaculatory function (6.55卤10.62 post-stroke vs. 8.68卤10.05 pre-stroke; p=0.0001), and sexual satisfaction (4.58卤3.36 post-stroke vs. 6.57卤2.98 pre-stroke; p=0.0001), increased significantly for erectile dysfunction (4.98卤3.35 post-stroke vs. 2.71卤3.58 pre-stroke; p=0.0001), but remained almost unchanged for orgasmic function (1.96卤2.91 post-stroke vs. 1.45卤1.89 pre-stroke; p>0.05).
CONCLUSIONS: In men with a history of ischemic stroke, there is a rather high prevalence of arousal disorders, desire dysfunction, erectile, ejaculatory, orgasmic dysfunction, and sexual dissatisfaction already before stroke suggesting associations between sexual dysfunction and stroke risk factors. After stroke, all evaluated types of sexual dysfunction and sexual satisfaction deteriorated significantly showing the impact of ischemic cerebral lesions on sexual function and processing. Although orgasmic function did not change significantly, reduction of the other sexual functions adds to a reduced quality of life after stroke.
