Title Influence of Sex Hormone Ratios on the Clinical Course of Multiple Sclerosis: A Case Study

Objective

Background BACKGROUND: Several line of evidence both in vivo and in vitro verify a probable correlation between sex hormones and Multiple Sclerosis (MS).

Design/Methods DESIGN/METHODS: During a prescheduled appointment, MS patients participating underwent blood sampling and evaluation of EDSS and MSFC clinical scales. The total duration of the follow up was 2 years and 85 MS patients (56 females, 44 RRMS) participated.

Results RESULTS: Testo/E2 and free-Testo/E2 correlated significantly with visual-motor coordination (9 Hole Peg Test-9HPT), and total functioning. Free-Testo/E2 correlated with cognitive function (PASAT test). The magnitude of the correlations was rather low ranging from 0.23 (Testo/E2-PASAT) to 0.40 (Testo/E2-9HPT). The direction of the correlations was positive, that is higher hormonal ratios are related with better function. Concerning prediction, Testo/E2 ratio was proven the best predictor of the aforementioned variables contributing significantly in their statistical prediction with adjusted coefficient of determination r2 ranging from 0.04 (prediction of PASAT variance) to 0.14 (prediction of 9HPT variance). There were no significant results concerning Prog/E2 ratio.

Conclusions CONCLUSIONS: Hormonal ratios maybe a useful predictive index of MS clinical disability and the natural history of the disease. We hypothesize that androgen-estrogen cross-talk and possible altered function of aromatase may have neuroprotective action in MS. No apparent correlation of clinical scales with PROG/E2 may lead to different neuroprotective PROG mechanisms of action. Specification of all the sex hormone target cell types is considered vital in order to define the exact cause of sex differences in MS and take advantage of the possible sex hormone therapeutic properties.
